Advance Care Planning and Health Care Directives

Advanced Care Planning North Dakota - This group was formed in June 2013 to organize a statewide initiative to impove advance care planning in the state. This website offers some great information. NDMA is a participating member.  
Health Care Directives - Honoring Choices provides resources for health care directives.
